As a Rehabilitation Case Manager, you will play a vital role in our interdisciplinary team, working closely with patients, families, and healthcare professionals to achieve optimal outcomes.

About the Role

• Develop and implement individualized care plans to meet the unique needs of patients
  
• Collaborate with the healthcare team to identify barriers to care and develop solutions to achieve patient goals
  
• Communicate effectively with patients and families to ensure informed decision-making and seamless transitions
  
• Document patient progress and outcomes in accordance with hospital standards and regulatory requirements
  
• Participate in regular team conferences to review patient progress and adjust care plans as needed
  
• Contribute to the development and implementation of hospital-wide initiatives to improve patient outcomes and satisfaction
  
  Qualifications
  • Masters or Bachelors in Social Work or a related field
    
  • Minimum of 2 years experience in case management or a related field, preferably in an inpatient setting
    
  • Effective communication and interpersonal skills, with the ability to work with diverse populations
    
  • Basic computer skills, including proficiency in electronic medical records and other relevant software
