Job Description:
Description The Transition Coordinator's primary responsibility
is to deliver the most remarkable patient experience, in every
dimension, every time and understand how he or she contributes to
the health system's vision of achieving that commitment to patients
and families. The Transition Coordinator is an experienced nurse
who is able to provide 1:1 instruction and education to
patients/families who will and have experience a surgical procedure
in their area of focus (ex. cardiothoracic, orthopedic). She/he
will serve as patient advocate, discharge nurse and provide follow
up calls to assess and assist the patient's transition home. She/he
will make appropriate referrals for follow up care and
rehabilitation (ex. cardiac rehab, physical therapy) and provide
ongoing continuity between the patient, their surgeon and their
primary care physician until the transition of care back to the PCP

a row. Required Qualifications:
- Bachelor's Degree Nursing.
- Upon hire: Washington Registered Nurse License
- Acute care experience in the specialty area assigned (ex
Cardiovascular CC, Orthopedic surgery, post-op care). Preferred
Qualifications:
- Master's Degree Nursing.
- Experience as appropriate to specialty (ex Cardiac
Rehabilitation, Out-patient cardiac nursing care, out-patient
orthopedics, PT)
