HEALTH OFFICER (CPHO) has an outstanding opportunity for a PROJECT
MANAGER - VALUE BASED CARE OPERATIONS.Work Schedule
- 100% FTE
- DAYS - HYBRID - 1 TO 2 DAYS ON SITEPOSITION HIGHLIGHTS
- Gain a deep understanding of various value-based care models,
specifically Making Care Primary.
- Work with a team committed to improving patient outcomes and
experiences by focusing on quality of care rather than the volume
of services provided.
- Employ experience working with diverse healthcare teams,
including physicians, nurses, and administrative staff, to
coordinate care and improve patient outcomes.
- Demonstrate ability to lead initiatives and manage change
within healthcare organizations, particularly in transitioning to
value-based care models.
- Engage with patients and their care, using tools and strategies
to improve adherence to treatment plans and overall health
outcomes.
- Participate in implementation of innovative solutions to
address challenges in value-based care, such as using telehealth or
remote patient monitoring.
- Ensure continuous learning and improvement, staying updated
with the latest trends and best practices in value-based
care.DEPARTMENT DESCRIPTIONThe Office of the Chief Population
Health Officer (CPHO) is charged with system-wide population health
and value-based care strategy and integration. The office partners
with UW Medicine Primary Care Clinics, UW Medical Center,
Harborview Medical Center, Valley Medical Center, Fred Hutchinson
Cancer Care, Office of Health Care Equity, Digital Health and other
UW Medicine stakeholders, better linking clinical care, operations,
and contracting/payment strategies with other operational entities
related to value-based arrangements.PRIMARY JOB RESPONSIBILITIES
- Assist CPHO team members in managing all aspects of value-based
programmatic work, including tracking workstreams and
communications.
- Manage content development and strategic direction for key
stakeholders and work groups.
- Track the development and execution of pilots using established
process improvement methods.
- Coordinate development and maintenance/tracking of plans around
specific work.
- Manage assessment work by gathering data and coordinating the
analysis of data sets.
- Maintain, update, and communicate program progress updates or
variance to the program governance structure and identified
stakeholders.
- Create and coordinate materials for communication, education,
and highly reliable practices for operational teams and provider
groups.REQUIRED QUALIFICATIONS
- Bachelor's degree.
- 4 years' experience.
- Experience in integrated healthcare systems (hospital and
medical group).
- Demonstrated success in change management.
